About the role

The Cambridgeshire Learning Disability Partnership (LDP) brings together specialist health and social care services for people with a learning disability.

We have two exciting, permanent opportunities available within two of our locality teams.

  • Huntingdonshire Team based in Scott House, Huntingdon (one full time posts and one 21-30 hour post)
  • South Team based at Sunley House in Papworth (one 18.5 hour post)

The successful candidates will contribute to this unique model of practice as part of an integrated team alongside Health and Social Care Practitioners such as Adult Support Co-ordinators, Social Workers, Community Nurses, Occupational Therapists, Physiotherapists, Psychologists, Psychiatrist, Speech and Language Therapists and Art / Music Therapists.

The LDP are currently working to a hybrid model in line with our service needs. Following an induction period, our Social Workers will work from the office up to three days each week. This gives our Social Workers a quiet space and plenty of time for reflection whilst still balancing collaborative working across the multi-disciplinary team and peer support. It has also increased our Social Workers autonomy and has had a positive impact on wellbeing and work-life balance.

Cambridgeshire County Council has adopted a new approach to social work and social care to make it increasingly proactive, preventative, and personalised. Transforming Lives is a three-tier approach to working with individuals, carers, and families that consider holistic, creative solutions to enable individuals meet their aspirations through the care and support planning process.
