About the role
Flow and Capacity team (FACT): Undertake assessments; provide advice and co-ordinate support for people who are in hospital; discharged home with reablement services or who transfer from hospital to settings such as residential Discharge to Assess placements or the Recovery and Independence Bed Unit.
Adult Social Care Workers play a vital role in supporting the safety, dignity and well‑being of vulnerable adults. You will work with a wide range of individuals, including older people, adults with physical, sensory or cognitive impairments, and people with mental health needs, to include ASD.
The role involves helping social care and health care providers to identify and manage risks, responding to concerns about abuse or neglect, and offering guidance and support to those who need it most. You will work closely with a variety of professionals and partner agencies, always with the goal of promoting welfare and ensuring people receive safe, high‑quality care.
This is a supportive role at the heart of preventative work—building strong relationships with care providers, strengthening their safeguarding knowledge, and helping them deliver consistently good standards of care across the health and social care sector.
This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate. It is also a regulated activity and will be subject to a Children and Adults Barred List check. West Northamptonshire Council is committed to safeguarding and promoting the welfare of children and vulnerable adults and expects all staff and volunteers to share this commitment.
